Financial Assistance given to War Widows /Disabled Ex-servicemen and their

dependents from Army Central Welfare Fund (ACWF) by AG’s Branch, Army HQ, New Delhi.

2	Mobility Equipment	Disabled soldiers	Quotation, Date limb last fitted
3	Self Employment	War Widow & Boarded out War Disabled Soldiers	Project Report, Undertaking Certificate for utilisation of fund within one year
4	Daughter’s Marriage	Widow, Boarded out Disabled Soldiers and Ex-servicemen	Date/Place of marriage, Certificate by Gram Panchayat, Wedding Card.
5	Medical Treatment	Widows, Boarded out Disabled Soldier and Ex-servicemen.	Assistance from AGI/KSB. Medical case sheet, Bills/estimate of Hospital.

In addition to documents mentioned above, copies of the following documents are also required to be attached: -

a) Deceased/disability certificate.

b) Extract of relevant pages of discharge book.

c) Photocopy of bank passbook for last one year.

d) Copy of Pension Payment Order.

e) Details of all benefits received till date from all sources.

f) Photograph of beneficiary.
